SDC has different types of forms depending on your type of enquiry.

• To enrol via internet you have to complete all the details of the ENROLMENT FORM.
• To make any changes on your courses enrolled, fill in the AMENDMENT FORM.
• To book performances of the SDC Junior Dance Company, fill in the BOOKING FORM.
